WAIVER FORM

1- I fully understand and accept the risks involved in playing Lazerxtreme.
2- I release discharge and agree not to sue Lazerxtreme from any liability for injuries or incidents in connection with my participation in the game.
3- Guardian consent is required for players under 18 years old.
4- This waiver is valid for 7 days from the date of acceptance for continuous play.
